Required Documents for Registration and Tuition Information

Application for admission to Mother of Providence Regional Catholic School
2026-2027 School Year 
 
Those applying for Pre K3 must be three years of age by August 31, 2026
Those applying for Pre K4 must be four years of age by August 31, 2026
Those applying for Kindergarten must be five years of age by August 31, 2026
Those applying for First Grade must be six years of age by August 31, 2026

After acceptance, you will need the following items. Please use this list as a checklist of required documents. All documents must be submitted electronically at the time of enrollment.

• Birth certificate
• Baptismal certificate, if baptized 
• Current immunization record
• Pennsylvania Health Assessment CD 51 Form (updated annually PK3 - Transitional Kindergarten)
• Pennsylvania Physician’s Exam Form (updated annually Kindergarten and older)
• Pennsylvania Dentist’s Exam Form (Kindergarten, 3rd, and 7th grade only, or new to PA schools)
• Non-refundable registration fee per family

28 PA.CODE.CH.23 (School Immunization) requires students in all grades to have the following BEFORE the start of school:

• 4 doses of tetanus, diphtheria and acellular pertussis (usually given as DTP or DTaP)-(1 dose on or after 4th birthday)
• 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after the previous dose given)
• 2 doses of measles, mumps, rubella (Usually given as MMR)
• 3 doses of hepatitis B
• 2 doses of varicella (chickenpox)
